I mean the exclusions under Computer -> Antivirus and antispyware -> Exclusions in the advanced setup. These are applied by all scanners.

No, these exclusions are in the Antivirus section which means they are applied by all scanners.

 

There's no way to run a full on-demand scan if we have files and folders exclusions in there?

Then where do I go to set file and folders exclusions that will be applied to the real-time scanner only.

This is not possible, there are also other scanners besides real-time and on-demand scanners. You can only configure the on-demand scanner to ignore exclusions. Your goal can only be accomplished by selecting specific targets in the on-demand scanner.
