How does the vulnerability and patch management module work in an offline environment? Does ESET download updates to its server and then push them to client, or do the clients need to download updates from the internet through a proxy?

It doesn't. First of all, it requires ESET PROTECT (cloud). Also updates are downloaded from servers  of particular software makers.
