Lower device limit?

I would like to lower my device limit and I am aware that in order to do so I must contact customer support, however I cannot find the necessary form, on the hxxp://www.eset.com/us/ website, which is required to be filled out in order to contact the organization. My limit of devices for ESET Smart Security Premium is currently 3 and I would like to reduce it to 2 (while maintaining the subscription for my mobile device, as well).
